HISTORY OF THE DEPARTMENT

Department Ambulatory Care" was founded on January 5, 1999 by the order of the rector of Ternopil State Medical Academy named after I.Ya. Horbachevskyy. Organization of the first, within higher educational medical institutions of Ukraine, multidisciplinary department of Ambulatory Care and Family Medicine at the Medical Faculty of the University can justly be regarded as a significant contribution to the development of family medicine in our country. It has really become a department of ambulatory care - family medicine practice, where specialists of family medicine could be trained on the undergraduate level. In particular, the next courses are taught here: ambulatory-outpatient therapeutics, surgery, paediatrics, obstetrics-gynaecology, and functional diagnostics.


Professor Volodymyr Goshchynsky, MD, PhD, Doctor of Med. Sci., was the organizer of the Department and led the department until June 2012.

The initial staff of the Department

To the newly created department teachers from various clinical departments of the Ternopil State Medical Academy were transferred: Assoc. prof. V.B. Hoschynskyy (Department of Surgery, faculty of postgraduate education), assistant professors V. Pavlikovska (Department of Children's Diseases), O.O. Vorontsov (Department of Obstetrics and Gynecology), G.F. Chernyavska and R.A. Badyuk (Department of Hospital Therapy No.1), N.O. Yepishyna and G.V. Tytar (Department of Faculty Therapy), and laboratory assistant M.B. Masna (from the Department of Pediatrics, faculty of postgraduate education);  N.Ye. Botsyuk and I.O. Borovyk were hired on positions of assistant professor and senior laboratory assistant respectively. It was this team, that in the short terms (until January 16, 1999) had developed working programs and methodology instructions for the learning process for VI course subject of Ambulatory Care and IV and V courses subject of Functional Diagnosis. Also, the next persons took an important place in the history of the Department: Assoc.prof. I.I. Zhura, Assoc.prof. V.V. Pavlikovska, Assoc. prof. A.V. Zorya and Assoc.prof. S.M. Butvyn; assistant professors N.O. Arkhipova, R.A. Badyuk, M.V. Boychak, N.O. Yepishyna, N.V. Myhaylechko, Ju.P. Kuzjmych, I.F. Banadyha, G.F. Chernyavskaya, G.V. Tytar, all of them took an active part in the formation of "the Department of Policlinic and Family Medicine" and helped to make the Department one of the leading departments of family medicine in Ukraine.

According to the Order of the University number 318 of May 28, 2012, starting from  June 1, 2012 "Department of Ambulatory Care and Family Medicine with Medical Equipment" was renamed as "Department of Primary Health Care and Family Medicine", and Professor Liliya S. Babinets, MD, PhD., Doctor of Med. Sci., has become a new head of the Department.

The present staff of the department includes Professor Starodub E.M., MD, PhD., Doctor of Med. Sci., 6 Associated Professors: Irina O. Borovyk, Nataliya Ye. Botsyuk, Neonila I. Korylchuk, Oxana R. Yasniy, Alexander A. Vorontsov, Vadym M. Tvorko, Bohdan A. Mihenko, 1 Assistant Professors: Svitlana S. Ryabokon, laboratory assistant Mariya B. Masna. Faculty members of the Department are not only highly qualified doctors and experienced teachers, but also talented scientists.

 Academic Affairs

At the Department, students of the sixth course of medical faculty master their theoretical and practical knowledge. The Department teaches subjects of Internal Disease in Family Medicine, Pediatrics in Family Medicine, Surgery in Family Medicine, Obstetrics and Gynecology in Family Medicine. Successful implementation of training programs is possible because of the powerful clinical base of the Department.

The clinical bases of the Department are:

Teaching-practical centers of primary health care

Thanks to the built and well equipped teaching-practical centers of primary health care in the villages of Zarubincy (Zbarazh District), Hnylytsi (Pidvolochysk District), Hovyliv (Terebovlya District), Uvysliv and Kokoshyntsi (Husiatyn District), students of the sixth course have their independent weekly educational practice of the family doctor in real conditions. They perform functional responsibilities of family physician: perform outpatient reception of patients; conduct preventive examinations of the decreed populations with registration of all the received data (patients' records, family passport, decreed patient's records, etc.); they take part in anti-epidemic measures, in organization of health and educational work among the population, providing the first and urgent care to patients with acute diseases, injuries, poisoning and accidents. Students conduct ECG and laboratory examination of patients and can get on line advice of the relevant specialists from the University Hospital.

The aim of the educational process is the formation of clinical thinking in medical students. Studies are carried out in accordance with the programs approved by the Central Methodological Cabinet of the Ministry of Public Health of Ukraine; practicals are conducted according to the credit-modular system within the "One Day" regimen. Test control of students' knowledge in the «Moodle» system is embedded into the study process. A framework for Independent integrated test tasks semester exam. A scope of multiple choice test tasks was created for integrated semester exam. Teachers use methodological instructions, situational tasks and relevant cases from patients' records in the educational process. For better mastering the study materials, interesting cases that are difficult for diagnosis are discussed at clinical rounds of professors and associate professors. Aiming to improve the evaluation and registration of students' practical skills, Department staff has developed a "Protocol of Supervision of Ambulatory Patient"; on the cycle 'general practice - family medicine', each student fills in "Medical Chart of Ambulatory Patient." The Department implemented an independent individual research and educational work of students of the 6th course, educational-practical conferences are held regularly.

According to the requirements of the CMC of the Ministry of Public Health of Ukraine, the faculty of the Department in cooperation with the Department of Therapy and Family Medicine [Faculty of Postgraduate Training] has developed continuous program of mastering practical skills of students of the 6th course and interns of family medicine. The program is approved by the Ministry of Public Health of Ukraine and is mandatory for higher medical educational institutions in Ukraine.

With the participation of employees of the Department, publishing house "Ukrmedknyha" has printed manuals "Clinical phlebology" (2008) and "Venous thrombosis and its complications" (2010). Professor L.S. Babinets is the co-author of the handbook for teachers "Managing of physiological pregnancy" (2012). The staff of the department has prepared supplemented edition of the textbook "Family Medicine".

Scientific and Research Affairs

Research interests of the Department are focused of scientific developments in therapy and family medicine, surgery. A team of scientists of the Department, headed by Professor Lilia S. Babinets has developed new treatments for chronic pancreatitis on the background of concomitant diseases and study the problem of pathogenesis of polynutrient deficiency in patients with chronic pancreatitis. Surgeons of the Department continue the study, initiated by Prof. V.B. Hoschynsky, devoted to improving the immediate and remote postoperative results after abdominal surgeries. New and promising research direction in this regard is the use of lyophilized xenodermoimplants to prevent failure of intestinal sutures and anastomoses. Department staff is working to solve current issues related to pathogenesis, modern methods of diagnosis and treatment of varicose veins of lower extremities and the prevention of postoperative recurrence and arteries disease.

The staff of the department has introduced a significant scientific contribution to the study of the pathogenesis of secondary osteoporosis in various pathological conditions and its treatment.

Department researchers have developed a new concept of pathogenesis of polinutrient insufficiency in chronic pancreatitis, making it possible to optimize methods of treatment. They have proposed and implemented in practice methods of correction osteodeficiency in patients with chronic pancreatitis. It should be noted that the research staff of the Department were also devoted to the study of disorders of trophical status in patients with chronic pancreatitis. Based on these studies complex homeosyniatrical correction was implemented into practice. Important in terms of practical medicine was comprehensive study of lipid status in patients with chronic pancreatitis.

Prospects for scientific work are to improve interdepartmental planning and their own scientific developments and their implementation in practice.

It has becom the department tradition to organize the annual scientific conferences and training seminars for family doctors of Ternopil region, as well as district scientific-practical conferences.

 The main scientific achievements of the department

- Created the concept of polynutrient insufficiency in chronic pancreatitis;

- Proposed and implemented into practice the methods of correction osteodeficiency in patients with chronic pancreatitis;

- Formulation of thesis on the lipid status in patients with chronic pancreatitis and suggested combined treatment statins;

- Condition of microflora of colon was studied in patients with chronic pancreatitis in combination with COPD and method of correction of dysbiosis with the inclusion of symbiotics was implemented;

- Implementation into surgical practice new suture polymeric materials with antibacterial properties and ability to self-resorption: kaproyod, kapromed.

- The role of cytokine status in spinal osteochondrosis with reflex syndromes was revealed and methodology pharmacological and acupunctured correction of pain was implemented.

- The influence of concomitant hypertension and endothelial dysfunction in the pathogenesis of trophical changes in patients with chronic pancreatitis and the role of metabolic therapy of such conditions were investigated;

- Developed and implemented in practice the methods of sealing of veins using the adhesive composition CL-3.

 

Clinical Affairs

The main clinical base of the Department is at the Ternopil Municipal Polyclinic department No.2. Associate and Assistant Professors of the Department provide treatment and medical consultations within departments of the Polyclinics, as well as medical consulting of the patients in Ternopil Municipal Clinical Hospital No.2. Surgeons of the Department have their practice in the inpatient surgical department of Ternopil Municipal Clinical Hospital No.2. Family doctors and physicians of the department perform consultative admissions of patients in ambulatories of family doctor in the villages of Hnylytsi, Zarubincy, Kokoshentsi, Hovyliv, Uvysla and geriatric home in the village of Denysiv.

The faculty of the Department provide treatment-diagnosis services to the citizens of Ternopil; associate professors and assistants of the Department perform consulting admissions of patients in teaching-practical center of primary health care in the villages of Hnylytsi, Zarubincy, Kokoshentsi, Hovyliv, Uvysla and geriatric home in the village of Denysiv, accomplish urgent clinical duties in the clinic and at home.
